Our admissions team is available 24/7 to assist with any questions and facilitate a smooth transition to treatment.

For teens ages 12-18 needing intensive day treatment with educational support. Returns home each evening. 

 

Adolescent PHP Referral Form

 

For adults needing intensive daily treatment without overnight hospitalization. Mental health and co-occurring treatment tracks available.

Adult PHP Referral Form

For adults requiring 24/7 structured hospital care for mental health or substance use conditions.
