Clinical Resources Specialist


Description:  Development and coordination of a comprehensive aftercare/disposition plan. Assess and provide resources for patients who require community-based services. Participate in treatment planning meetings. Provide direct clinical services to assigned patients, inclusive of individual and family treatment, case management, disposition planning and case coordination to patients with mental illness, dual diagnosis, and/or addictions.

Qualifications:   Candidates must have strong organizational and interpersonal skills, and the ability to multitask in a fast paced environment. MSW is required; LCSW preferred.  Previous discharge planning experience or previous experience working on an inpatient program servicing adults with mental health and/or substance use conditions is desirable.
